You must complete the EOI application either within 30 days of the Qualifying Life Event that allowed you to apply for the coverage or within the dates of the Summer Enrollment period. The EOI application requests information about your and/or your dependent's health. Once completed, the employee must mail it directly to Fort Dearborn Life Insurance (FDL) for review and eligibility determination. Give daytime telephone numbers where you can be reached. FDL may ask you to have a medical examination and/or request your medical records as part of the review process. For information on the status of your application, call FDL at 800-451-0271. If no action is taken on your application within six months, it will be cancelled and you must reapply. This may require that you have another Qualifying Life Event or wait for the next Summer Enrollment. You will either be approved or denied coverage based on the medical information provided by you and/or your doctor. You will receive an approval or denial letter from FDL. To add coverage that is approved, notify Human Resources at CPC by providing a copy of the approval letter. Approved coverage will be effective the first of the month following the date your approval letter is received in Human Resources at CPC. The earliest effective date for Summer Enrollment changes--without a Qualifying Life Event--is September 1st. To start the EOI process, complete the EOI application and mail it to the address on the application. All employees with address changes must complete the Collin College Change Form and send it to Human Resources at CPC. HR will in turn update or notify the proper place to update HRS (HR/Payroll), SIS, FRS, ERS, and TRS. In addition to completing the Collin College Change Form, a full-time employee in ORP instead of TRS must contact his/her ORP company with the address change. In addition to completing the Collin College Change Form, part-time employees with MetLife PERC accounts need to contact MetLife directly with the address change. ...Change your beneficiary(ies) with ERS, TRS/ORP To update your beneficiary data with ERS, go online at http://www.ers.state.tx.us. Log into ERS OnLine, request a new password if necessary, and change your beneficiary(ies) online. ERS will mail you a confirmation form to sign and have witnessed. Important: The beneficiary change is not official until after the signed and witnessed form is received at ERS. To update your beneficiary data with TRS, complete the TRS Designation of Beneficiary Form and send it to Human Resources at CPC for forwarding to TRS, or mail it directly to TRS. To update your beneficiary data with your ORP company, contact your ORP company to request a beneficiary change form.
