Leave Benefits

General Leave Benefits information can be found in the Leave and Absence Guidelines:

Collin College full-time employees have the following types of leave benefits available to them:

                                                                            

 

IMPORTANT:  Leave Without Pay (LWOP) may not be elected by employees or approved by supervisors. Collin College does not offer LWOP as an optional form of time off.  Time off is meant to be granted and taken within the existing leave types and policies. Requests in excess of available leave should be denied by supervisors.  Exceptions to policy always first require review by the appropriate Vice President and then approval by the President, if recommended by the VP.
 

Leave Balance Priority: All eligible paid leave must be used before leave without pay is permitted.

Leave of Absence (Medical): Credit vacation may not be used during a leave of absence for medical reasons

Leave of Absence Forms 

  • Application for Leave Form (Multi-copy form available from your department or HR)

  • Time Sheet (Required of all part-time employees and all full-time non-exempt    employees)

  • See below for additional forms for FMLA-covered absence

 

FMLA

 

Advance Notice of Leave of Absence:  Notify HR in advance of any family or medical-related absence of more than five consecutive work days (covered under the Family Medical Leave Act), when foreseeable.

*Current medical release is required before you can return to work.

*A statement of disability from your doctor, stating that you are medically unable to work, with a start and estimated end date is required within five days of the beginning of your leave.

*At the end of the leave, a written release from your doctor is required.

*You must use all eligible paid leave first, and then would be in a leave without pay status (LWOP), if necessary, for the remainder of your leave.

*Your group insurance will continue in effect during your FMLA leave just as it was while you were actively at work, if you continue to pay the employee portion of the premiums.

*If premium payments are not made on time, all coverage except employee health will be canceled.

*New dependents may be added by submitting insurance forms within thirty days of eligibility.

*If you are unable to return to work at the end of your available leave, your employment will end.

Restricted Leave Status

Employees may placed in a restricted leave status for excessive absenteeism or a pattern of absences or abuse of leave. The restricted leave status would result in restricted use of leave balances, requirements for additional documentation, and would be grounds for leave without pay or other disciplinary action up to and including termination of employment.
